If you're used to being able to stand toe-to-toe with enemies and dish out damage, you'll be at an extreme disadvantage with this game because that's just a quick way to die.

As the above poster pointed out, the trick is to get hit as little as possible. Get used to rolling! Throwing a point into Footwork helps, too--you'll roll farther. Try to get behind your enemies--you take double damage if you're hit in the back until you throw a point or two into Position , but so do your enemies, so take advantage of that.

Quen is useful, but I find that Yrden is equally useful--especially in the beginning of the game--on Hard difficulty and above. You can hold an enemy and get behind them and pound on them, or take a strong enemy out of the fight while you concentrate on weaker ones.

The combat takes some getting used to, but if you tackled Dark Souls, you can manage this.
